Coachella Valley Community: Mental Health and Substance Abuse
Thank you for participating in this survey on mental health and substance abuse within the Coachella Valley.
This survey is being conducted by HARC (Health Assessment and Research for Communities) on behalf of the Betty Ford Center. Both organizations are nonprofits located in the Coachella Valley.
We are conducting the survey so that we can better understand the health needs and barriers to care for those with mental health and substance use conditions. The survey covers areas such as demographics, healthcare access/utilization, mental healthcare, and substance usage.
You can skip any questions that make you uncomfortable, and you can quit the survey at any time.The survey will take approximately 20 minutes to complete.
At the end of the survey you will have the opportunity to enter a drawing for a $100 gift card. Your responses to this survey will be confidential. That means that if you choose to enter your name or contact information for the drawing (you don't have to), it will never be tied to your responses and no one will ever connect your name to what you said. All results will be reported anonymously. Trained researchers will handle your data with care to make sure all of your information is protected.
