Stroke Prevention Awareness Survey

Different Strokes is working with Dr. Indira Natarajan (Consultant Physician Stroke at Royal Stoke University Hospital, and Clinical Director for Stroke, Integrated Stroke Delivery Network West Midlands) and Medtronic, on secondary prevention of stroke.
This survey is only for stroke survivors.

A separate survey for healthcare professionals will be circulated in the near future.

2.When did you have your stroke?(Required.)
3.How old were you when you had your stroke?(Required.)
4.Where do you live?(Required.)
The next questions are about advice you may have been given to prevent another stroke.
5.Are you aware of the ‘Know Your Numbers’ campaign?(Required.)
6.Has a healthcare professional provided information to you about healthy eating and salt reduction?(Required.)
7.Has a healthcare professional given you advice about exercise, such as walking or other activity for 30 minutes per day?(Required.)
8.Has a healthcare professional given you advice about not smoking or quitting smoking?(Required.)
9.Has a healthcare professional given you advice about reducing alcohol to less than 14 units per week?(Required.)
10.Has a healthcare professional given you information about your target blood pressure (should not exceed 130/80 mmHg)?(Required.)
11.Has a healthcare professional given you information about your target HbA1c (should be less than 53)?(Required.)
12.Has a healthcare professional given you information about your target LDL cholesterol (bad cholesterol, should be below 2)?(Required.)
13.Have you been screened for Atrial Fibrillation?(Required.)
14.What percentage of strokes do you believe are caused by Atrial Fibrillation?(Required.)
