2026 City of Stratford EarlyON Parent/Caregiver Survey

Thank you for using City of Stratford EarlyON programs and services. We’d like to gather some thoughts and ideas from you.

The survey should take approximately 10 minutes to complete. The answers to all of the questions in the survey will be kept confidential. Any question you are not comfortable answering may be left blank and you can stop the survey at any time.

Participating in the survey by filling out the questions will be considered your consent to participate.

Upon completion of the full survey you have the option to have your name entered into a random draw for a chance to win one of five $50 President’s Choice gift cards.

Note: In this survey we use the language "parent" to refer to a person who has primary caregiving responsibilities for the child attending the EarlyON.
