Who can complete this survey?
This survey is intended for parents or primary caregivers of children in grades 3–6. We only need one parent or primary caregiver to complete the survey for each child.
When do I need to complete this survey?
Please complete this survey within 5 school days.
Why am I being asked to complete this survey?
Your school has been participating in the Kids Active Travel Program, which is funded by Sport and Recreation Victoria who want to understand if the Program is working.
What is the survey about?
This survey will ask about your child's travel habits, physical activity levels, and your views on active travel. It takes less than 10 minutes to complete and is designed for one child. If you have more than one child in grades 3–6 at the school, please complete a separate survey for each child. This ensures that we accurately capture each child's experiences.
The survey is being run by Sport and Recreation Victoria, part of the Department of Jobs, Skills, Industry and Regions (the Department). The survey has received Research in Schools and Early Childhood Settings (RISEC) approval, which is managed by the Department of Education and Training.
Is participation voluntary?
Your involvement in this survey is completely voluntary. Your decision not to participate will not affect your relationship with the school. You may withdraw at any time.
If you wish to withdraw, please do so within one month of completing this survey by emailing getactive@sport.vic.gov.au
How will my privacy and confidentiality be protected?
Data collected through this survey will be used to understand the impact of the Kids Active Travel Program and to guide potential future investments in the Program.
The survey is not intended to collect information which will be used to identify you or your child. The survey is hosted by Survey Monkey and will be stored securely by the Department.
The Department will store the data on a secure server for a minimum of 5 years to use to evaluate the Program, after which time the data will be destroyed.
The Department may share the data with a third party for the purpose of analysis. Any publications created using this data will not identify individuals.
The Department is committed to protecting your privacy. Any personal information collected, held, managed, used, disclosed, or transferred will be held in accordance with the Privacy and Data Protection Act 2014 and other applicable laws.
You may contact the Department to request access or correction of your information, or for other concerns regarding the privacy of your personal information, by emailing the Department's Privacy Team at privacy@ecodev.vic.gov.au. The Department's Privacy Policy is also available by emailing the Privacy Team.
