Thank you for taking part in Scottish Book Trust's Scotland's Stories creative readers' journal project! We are really looking forward to seeing how you have used the journal to respond to Scotland's Stories.

Firstly, we will ask you to confirm in what ways you are happy for us to your use your contribution.

On the next page we will ask you to please upload a photo of each of your completed activities.

On the final page we will ask you to complete a short demographics questionnaire. This will help us to see if there are common differences in how people completed their journals across demographic groups. You do not have to complete this section if you would prefer not to.

We will keep your responses to this survey safe, secure and confidential. Read our privacy policy for more information.
