Any adult who is a parent, guardian, caregiver or otherwise cares for a child or children AND is affiliated with Medway, MA.
Who is conducting this survey?
‘SUP Medway is a Substance Use Prevention coalition with the mission to engage in collaborative community work focused on the reduction and prevention of youth drug use, including alcohol and vaping.
What is the purpose of this survey?
The coalition is conducting a community assessment to fully understand the problem of youth drug use as well as existing protective factors and resources that reduce the likelihood of youth using drugs. This information gathering is to ensure that the coalition’s prevention efforts will effectively address the local challenges and reinforce the protective factors to keep and improve Medway as a healthy and safe community, especially for youth.
How long does this survey take?
The average time for completing this survey is less than 10 minutes.
What do I get in return?
As a thank you for participating, you have the option to enter a drawing for a gift card:

When the survey is closed, participants will be randomly selected to receive a gift card. In addition, your honest responses help the community coalition understand the needs and strengths already here in Medway.
Are my responses anonymous?
Yes, this survey is completely voluntary and your answers are anonymous. While your honest responses are very valuable for the purposes of youth drug prevention efforts, you may skip any question you are not comfortable answering. No personally identifiable information is collected unless you wish to be entered for a drawing of a gift card as a thank you for your participation. If you enter your contact information it will NOT be connected to your survey responses. There will not be any penalties, charges, or consequences for your honest responses; rather, the goal of this survey is to get population-wide information such as norms, themes, group perceptions, and trends.
