2025 S.L.A.A. Membership Survey

Dear Fellowship Member,

Welcome to the updated 2025 edition of The Augustine Fellowship, S.L.A.A., Fellowship-Wide Services, Inc. (F.W.S.) global membership survey. Your participation is essential to enabling us to continue working to ensure that S.L.A.A. becomes more accessible, relevant, and helpful to you and the addicts who still suffer around the world. The Fellowship is very grateful for your participation and giving Sex and Love Addicts Anonymous (S.L.A.A.) an opportunity to better serve you and everyone.

The survey will ask you demographic questions, questions about your experience of the program, and questions regarding sobriety and the impact of S.L.A.A. on your life. All answers are voluntary. We suggest answering all the questions, however please answer only those questions you are comfortable with. You have the right to withdraw from the survey at any time by closing the web browser.

Please know S.L.A.A. is committed to the protection of data gathered through this survey. To accomplish this, the Board Outreach Committee (BOC) will gather the data in compliance with the HIPAA laws of the United States by not collecting personal information such as insurance, medical data, or IP addresses. The Fellowship engages in full compliance with GDPR standards in the European Union by not collecting personally identifiable data such as name, location, and phone number. All data gathered through survey monkey also adheres to standards of encryption and security. Only aggregated and anonymous data will be available to the Trustees, Board Committees, and eventually, the Annual Business Meeting (ABM) Conference Committees. F.W.S. will institute policies to ensure anonymity.

We ask for your help in making everyone you know in S.L.A.A. aware of this survey. This will help us obtain more accurate information and better enable the Trustees and Board Committees to develop more precise resources for the Fellowship, the Addict who still suffers, and society as a whole. This includes guiding the development of more supportive resources for our members.
