Thanks to funding from NMDOH, NMASBHC is offering 4-6 awards of up to $75K each for SBHC planning and special project expansion. This application is for funds to be used in FY27 (July 1, 2026-June 30, 2027).

Level 3 SBHC Planning Grants will allow grantees to assess and plan for new SBHCs or expansion of SBHC services. This can include new brick and mortar SBHCs, new telehealth services, new mobile SBHC services, new services at existing SBHCs, etc.. Level 3 SBHC planning grants are available to previous SBHC planning grantees, current SBHC medical sponsors, or medical sponsors able to take over SBHC operations in locations previously closed.

SBHC Level 3 Planning Grant Narrative Questions
  1. Describe your school-based health center planning efforts for the proposed SBHC or expansion of services to date. Include key accomplishments and any challenges encountered.
  2. How have you engaged school leadership, staff, parents, youth, and community stakeholders in your planning process thus far? How will this funding help move your efforts forward?
  3. What community conditions support the need for a new SBHC or expansion of services? Consider factors such as student health needs, school or parental requests for SBHC services, uninsured or underinsured populations, access to local health providers, behavioral health capacity, and existing school health infrastructure.
  4. List and describe your current and potential partners. Explain their roles and how you are collaborating or plan to collaborate to support SBHC planning, coordination, and long-term sustainability.
