1.Your Name and guest's name, address, phone number, e-mail address. Are you a Beth Am member?(Required.)
2.Would you like to host guests for Seder or be a guest?(Required.)
3.If you can host, how many people can you host on WEDNESDAY April 1; how many on THURSDAY April 2
4.Guests: How many people are in your group including yourself on WEDNESDAY/THURSDAY?(Required.)
5.Hosts: Are there pets in your home? Guests: Are you allergic to any pets?(Required.)
6.Ages of any children  who will be in attendance
7.Does your family have special food observances or allergies?(Required.)
8.Hosts: Are stairs required to enter your home? Guests: Can all members of your group use stairs?(Required.)
9.For Hosts and Guests: What would make a Passover Seder feel comfortable and meaningful for you (and your family)?

10.Guests: Does the host need to be within walking distance to your house? And how far would you be willing to walk? Would you be able to drive to your host? *Due to the limited number of hosts within walking distance, please inform us if you are possibly able to drive to a seder. It is possible that we will be unable to find a suitable location for everyone. We will do the best we can.(Required.)
