This form is for:

1) Any compliance area you answered "NO" to on your "Nazarene Safe Annual Update Compliance Questionaire," set a goal date for compliance and note the status of your current efforts, barriers to compliance, or ways a District Resource Coordinator (DRC) could help you.
2) Once completed, please have your plan approved by your Church Board, then forward a copy to the DRC.
