The information you supply on this form will be used by the Diocese of Limerick to administer and deliver the REFD programme and classes. The information on the form will be retained by the Diocese for the duration of the programme and for one year afterwards, after which it will be confidentially destroyed. You/Your child’s data will be processed under Articles 6(1) and 9 (2) (1) of the GDPR. By completing the form, you are consenting to the Diocese collecting, processing and retaining the data in the form in the manner outlined. Please note that this form is also processed by Survey Monkey. By submitting the form you also agree to the information in the form being processed by Survey Monkey.
