Nomination Form
Nominate an individual, team, program, or organization for an AzLA Service Award by completing this form.
Each nomination will require:
- A detailed written description of how the nominee meets the criteria for the award.
- One letter of support from an additional source. Additional letters of support are encouraged, but not required.
Please note: AzLA membership and/or an MLIS degree are not required to nominate or receive a service award.
The deadline for nominations is Monday, August 31, 2026.
