Opt-In for Employer Payroll Deduction of CAMRT Membership Dues through Horizon Health

By completing this form, you are indicating your interest to have your 2027 CAMRT/CAMRT-ATL membership dues collected by payroll deduction through your employer (Horizon Health) during the 2026 year.

Dues will be collected starting in February 2026. A total of $344.25 will be collected and put towards your 2027 CAMRT/CAMRT-ATL Full Practice membership. ($344.25 = $223.50 national dues + $51.50 CAMRT-ATL dues + $28 PLI + tax). Please note these are dues being collected for the next membership year.

Please note, should you go on leave during the payroll collection period and your employer is not able to collect the full $344.25, the collected funds will be returned to you by your employer and your 2027 CAMRT membership will not be processed. In this instance, should you still want a 2027 membership, you will need to contact CAMRT directly for purchase.

Notice: Payroll deductions will automatically continue each year unless you opt-out directly through your employer/payroll department.
