8/4 Memorial Public Survey

A committee has been formed to develop a memorial to recognize the impact of the Oregon District mass shooting.  It is important that we receive input from victims, survivors, employees and residents in the Oregon District, as well as first responders, community members and anyone else who was affected by the incident.  Your feedback will help to guide the committee as we work to create a space for healing.

Thank you for participating in our survey, we appreciate your willingness to share your feedback. This survey should take less than five minutes to complete. Questions with an asterisk require an answer.
