Please follow the directions and answer all questions listed to the best of your ability. These questions have no right or wrong answers since they reflect your personal experiences. The questionnaire should take about 18 minutes to complete.
Some of these questions may pertain to sensitive topics that may be uncomfortable to answer. Your identity will remain anonymous, and your responses will remain confidential. These responses will only be seen by this questionnaire’s creators and will not be released to outside sources.
