This questionnaire asks you to reflect on your recent phone call to the Good2Talk helpline. The purpose of this questionnaire is so we can learn how the helpline is working and how we can better support you. Your participation in the questionnaire is voluntary and you can stop at any time. There are no expected risks however if you do find yourself needing support (e.g., feeling upset) you can skip the question, stop the questionnaire or call Good2Talk (1-866-925-5454).

We ask that you complete the questionnaire and return it to us by clicking ‘submit’ at the end. Your responses are anonymous and confidential. We don’t collect or display any identifying information such as name, email address, or telephone number. You will not be identified in any publication or presentation of this evaluation.
