Introduction
Parents and Caregivers for Mental Health (PCMH), the family partnerships program of Children's Mental Health Ontario (CMHO), is conducting a province-wide evaluation to better understand how our peer support programs, resources, webinars, and partnerships are supporting families across Ontario.
This survey is intended for parents, caregivers, clinicians, and service providers who are connected to or familiar with PCMH, even if you have not connected with our services. Based on how you identify at the beginning of the survey, you will be directed to questions relevant to that role and experience. Please select the option that best reflects the perspective you most identify with regarding the PCMH program.
Your feedback will help us:
- Understand the impact of PCMH programs and supports
- Identify gaps, barriers, and opportunities for improvement
- Improve the accessibility, inclusivity, and usefulness of supports and resources
- Strengthen advocacy, partnerships, and future program development
Participation is voluntary, and responses will be kept confidential and summarized together, not individually. All questions are optional, and you may select “Prefer not to answer” or “Not applicable” as you wish.
The survey should take approximately 15 minutes to complete. Participants may also choose to enter a draw to win one of two $50 gift cards. Entering the draw will not connect your survey responses to your identity; participation will be handled through a separate link provided upon survey completion.
