Welcome to Ottawa's Primary Care Census!

To have an accurate understanding of primary care in Ottawa, we are working to reach every family doctor, nurse practitioner, and clinic director or office manager in Ottawa. Thank you for taking 5-10 minutes to ensure that you and your practice are counted!
The main goals of this survey are to:
  • Establish a comprehensive local database of family physicians and nurse practitioners in Ottawa.
  • Conduct a needs assessment for primary care capacity planning in Ottawa.

Who should participate?
  • All family doctors and nurse practitioners with patients in Ottawa.
  • All primary care office managers and clinic directors in Ottawa.
How will your response data be governed and protected?
  • Ottawa OHT-ÉSO will oversee the governance and use of collected data.
  • Any basic information you choose to share (e.g. name, practice name, etc.) will be separated from all other responses (e.g. roster size, plans for your practice) to protect any sensitive information shared before being stored on Ottawa OHT-ÉSO's secure SharePoint.
  • Providing your basic information will not add you to any distribution list, unless you choose to join our Primary Care Network.
  • All survey findings will be deidentified, aggregated, and suppressed as needed to protect your privacy before being used and shared regionally for system planning.
UPDATE As of July 8, 2025, if you indicate that you or your clinic are attributed to Ottawa West Four Rivers OHT (OWFR), your complete survey response will be shared with OWFR via encrypted file. OWFR OHT will store this data in their secured Client Relationship Management (CRM) system and agree to uphold the above commitments regarding the governance and protection of your data.
