Guidelines for the Nomination

Thank you for your interest in the OEDA Annual Awards process. You may submit multiple nominations, but each must be on a separate form.

5.Nominator Name and Organization
6.Describe the Nominee. (Nominee refers to the person, organization, partnership or initiative being recognized.)(Required.)
7.Explain how or why you feel that this Nominee is deserving of recognition. You may also upload supporting documents below.(Required.)
8.What results has this nomination produced? Where possible, describe the measurable impact on Ohio, a region, or a community.
