Healthwatch Leeds would like to find out how people who have experienced mental health challenges (and the people who support them) have found the change to NHS 111 for mental health crisis support. We know that crisis has a different meaning for each person. By crisis we mean when you or someone you know no longer feels able to cope or be in control of the situation. Any information you share with us will be used anonymously in a report that will be shared with people who provide services and made available to the public. 'Anonymously' means that we will not use any information that would identify you.To complete this survey you must live in Leeds, or have been in Leeds when the call was made. It should take about 5-6 minutes to complete and will help us to improve mental health services for people in Leeds.
