Applicant Partnership Assessment Form

The purpose of this optional survey is to help potential applicants for Round 2 of the Extreme Heat and Community Resilience Program (EHCRP) identify and connect with project partners. Fill out the survey to share information about your organization’s type, service area, expertise, and capacity and see responses from other organizations that are interested in applying.

Sharing your organization and project information with other applicants allows potential partners to view your responses and independently connect to explore partnerships. It also highlights opportunities for collaboration across different organization types and helps applicants meet EHCRP’s Lead and Co-Applicant requirements while building strong, well-rounded project teams.

Responding to this survey is voluntary. If you consent to share your survey responses, your contact information and answers will be added to a list that is publicly accessible via a link that may be shared via email, newsletter, or on LCI’s website.
