We are the National Academy for Social Prescribing (NASP) and the Patients Association.

NASP is a national charity that champions social prescribing, connecting people to non-medical activities, groups, and services to improve their health and wellbeing. We support communities by building partnerships, boosting investment, and raising the profile of social prescribing.

The Patients Association aim to amplify patient voices and ensure they are heard, acted on and embedded in how healthcare is delivered. We listen to patients, translate their experiences into evidence, and use that evidence to influence policy, and hold systems to account.

Together, we are surveying anyone who has ever had support from a Socal Prescribing Link Worker, so we can learn about your experiences. With this information, NASP can ensure its strategy supports the provision of the best and most appropriate social prescribing services for all.

These questions will ask you about social prescribing and your social prescribing link worker.

Social Prescribing connects people to activities, groups, and services in their community to meet the practical, social and emotional needs that affect their health and wellbeing. GPs, hospitals, social care services, local charities and others refer people to a Social Prescribing Link Worker.

A Social Prescribing Link Worker is someone who spends time with you to understand your needs and co-develops a plan with you to connect to non-medical support, improving your health and wellbeing, and to your community. They may also be called Community Connector, Social Prescriber or Community Navigator.
