The Prevention, Recovery, Resiliency and Mitigation (PRRM) Division is surveying the public to assess the extent of storm damages to private property in Mendocino County due to the 2022-2023 Winter Storms.
If you are not comfortable using this survey format for damage reporting, please contact the PRRM Division at: or by calling the disaster recovery hotline at: 707-234-6303 and leave your name, address, contact information and brief description of the damages.
  • Responses to this survey will help the County of Mendocino document the extent of damages to private property and support the pending request that individual assistance be made available by State/Federal Partners. Accurate, complete survey responses are critical to the County effort to secure Individual Assistance for residents as damages must be documented and verifiable by FEMA damage assessment teams.
  • Please note: At this time, individual assistance has not been made available by the State or Federal Governments and this survey is for damage tracking and documentation purposes only.
  • This is not an application for assistance. The purpose of this document is to gather damage information in order to assess the level of assistance needed.
  • Responses to this survey will not be shared with outside agencies unless you elect to share your information by selecting yes to question 14.
