This form is for the Excellence Award category.
This category recognizes a Hispanic individual who has demonstrated leadership and/or made significant contributions to a particular community, cause or field of work.
Below are the qualifications for an individual to be considered in this category. Only an individual who meets these requirements should be nominated for this award.
Nominees must:
- Be of Hispanic descent
- Reside and work in the Central Florida region
Special Notes:
- If the nominee is a business owner, they must demonstrate positive revenues through taxes or CPA-certified financial statements for 2025.
- Nominees in this category are not required to be associated with Prospera or the Hispanic Chamber.
- Founders or executive directors of nonprofits may apply in this category.
Those who do not qualify include:
- Board members, employees, and immediate relatives of employees of Prospera or the Hispanic Chamber
- Members of this year's Don Quijote Award selection committee
- Elected officials or those running for office
- Past award recipients. If the nominee has been a finalist for the Don Quijote Award in the past, but has not won, the nominee may apply.
A nominee may not submit an application in more than one category.
