1. If you are 60 years of age and older, book online at https://covid-19.ontario.ca/book-vaccine/

2. If you are Indigenous and under 60 years of age (including household members), call Hamilton Public Health’s COVID-19 Vaccine Hotline at (905) 974-9848, dial option 7.

3. Upcoming Pop-Up Clinics: To be announced.  None confirmed at this time.

If you live in Brant:

Booking Options:

1. If you are 65 years of age or older, you can book an appointment online at https://www.bchu.org/covid19bookageplus

2. If you are Indigenous and under 65 years of age (including household members), you can book an appointment by calling Brant County Health Unit ay 519-753-4937 ext.438 OR you can complete this survey and an online booking link will be emailed to you.

If you live in Niagara:

Booking Options:

1. Upcoming Pop-Up Clinics: April 21, 22, and 28 at the Mac Bain Centre in Niagara Falls. If you are Indigenous over 18 of age or a household member that is over 18 years of age, please complete this seperate survey https://www.surveymonkey.com/r/MacBainCentrePopUp and we will share your information with public health. You will be contacted to inform you of your appointment date and time.

2. If you are 60 years of age and older, book online at https://covid-19.ontario.ca/book-vaccine/

3. If you are Indigenous and 18 years of age or older (including household members), and would like to access any of the Immunization clinics in Niagara, please complete this survey and we will share your information with public health. You will be emailed a unique booking code with directions on how to book at a clinic near you.
