Before you fill out the electronic form below, we suggest you explore our nominations guide prior to submitting your nomination. The nomination form will guide you to the appropriate questions for the category you wish to nominate your candidate. Our system does NOT allow you to save your submission if it’s only partially complete. You’ll see in the form that we ask 2 questions for each category with responses that are no longer than 350 per question.
