Public Consultation Survey
Background
Transitions of care is a period of high risk for medication errors and miscommunication, that can lead to patient harm. Acknowledging the shared responsibility and accountability for high quality medication management at transitions of care (MM at TOC), the Commission has developed a DRAFT MM at TOC Stewardship Framework (the Framework) based on:
- Findings from a dedicated literature review and environmental scan
- Existing stewardship frameworks and programs, such as Antimicrobial Stewardship
- Expert advice from a dedicated project advisory group and a targeted consultation.
The Framework outlines key elements that hospitals, in consultation with primary care teams, can steward to improve MM at TOC. The Framework is composed of:

The Commission is seeking feedback on the draft Framework from health organisations and health professionals working in acute, primary and aged care settings via this survey.
The Commission is grateful for your time and experience, which will help inform the FINAL Framework.

The survey will take approx. 20 minutes to complete and will close on Sunday 27 October 2024 at 11:59pm (AEDT).
For the purpose of the Framework publication and this survey, the word ‘Individual’ refers to patients, their family, and/or carer.
