Please ensure that your abstract and presentation dive deep into innovations, practical solutions, evidence-based practices, quantifiable results, and key findings. 
Please keep in mind that this is not the place for a “community health 101” presentation.

Describe how your topic contributes to one or more of the conference objectives. Supporting data and specific cited resources are appreciated. 

Conference Objectives:
  1. Describe the impact of policy changes on safety net providers and underserved populations in Minnesota, and their effect on access, quality, and reimbursement.
  2. Identify strategies and resources to help adapt to the challenges of operating within our changing environment.
  3. Recognize the role safety net providers and community organizations play in promoting health equity, preventing and managing chronic disease, and ensuring access to the underserved.
  4. Showcase a variety of interventions to improve health and reduce disparities.
