Thank you for taking part in the Central Goldfields Shire Council COVID-19 Community Health and Wellbeing survey. We understand that this time has been extremely challenging for many members of our community, so we have designed this survey to understand the impacts on people’s health and wellbeing. We hope that the information provided will help guide our local recovery phase and inform local services of the challenges our community faced, and continues to face, during this time.
 
It's possible some people could find some questions sensitive, we can reassure you that all answers are anonymous.  In any case, if you'd rather not answer a question you can always leave it blank. We estimate the survey will take no longer than 10 minutes, unless you choose to write a lot in open-ended comment boxes.
