2026 OSSM Summer Middle-School Math Workshops (Updated) Survey

We look forward to having your student at our Middle-School Math workshops! Please fill out the form below to allow us to place our student in their class. If you have more than one student to sign up, fill out the form again for each individual student that you wish to sign up for workshops.

If the class that fits your student's needs is full (indicated below) please still register your student; they will be placed on a waitlist. Waitlisted families will be contacted 2 weeks before workshops as to whether a spot has opened up for that student.

NOTE: Unlike in previous years, you will sign up for the entire three-day week of workshops rather than selecting individual days. We would prefer that a student attend all of the days within the week you sign them up for.
