Celebrating Service and Dedication
The Arizona Health Care Association Awards program honors the exemplary service and devotion of long term care leaders, staff, residents and volunteers who demonstrate their commitment to quality on a daily basis. Please take the time to nominate those in your building who deserve an award.
Important Information
- The submission deadline is 5:00 p.m., Friday, April 17, 2026. Applications received after this time will not be considered.
- The Annual Awards Presentation will be held on Monday, June 15, 2026, at 6:00 p.m. during the AHCA Awards Gala at the Renaissance Phoenix Glendale Hotel & Conference Center.
- Each award winner will receive one free registration to the AHCA Annual Convention and the Awards Gala.
- If selected to receive an award, three high-def digital photos of the recipient will need to be submitted.
