This survey is designed to obtain your feedback on local provision of adult social care (Live Well and Age Well), particularly in relation to Accommodation Based Care and Community Based Care. We are keen to learn more about:
What support people use now
What support people may need in the future
What works well
What needs to improve
Where there are gaps in services
How to shape our next steps for the new All Age Accommodation Support and Community Based Care Frameworks.
