Lakelands Public Health Strategic Planning Survey

Lakelands Public Health (LPH) is developing a new Strategic Plan, and they want to hear from you. As a valuable member of the community, your ideas and opinions are important as Lakelands Public Health determines their focus and priorities for the coming years.

This survey will take approximately 10 minutes to complete and is available until June 30th. Questions are mainly multiple choice and easy to answer. There are a few questions that allow space for written feedback. Survey results will be used to inform organizational priorities.

Privacy
The protection of your privacy is important. This survey is anonymous and is being conducted by a third party. There are no personal identifiers requested during the survey. Survey completion is completely voluntary, and the results will be provided to Lakelands Public Health as grouped data. Please do not include your name, address, phone number or other personal identifiers in any of the comment boxes. Excerpts from anonymous comments may be included in a summary report; however, all identifying information will be removed by the third party. Lakelands Public Health (LPH), formerly known as the HKPR District Health Unit and Peterborough Public Health, is pleased to provide important public health programs and services to residents in the City of Kawartha Lakes, the County of Haliburton, Northumberland County, the City and County of Peterborough, Alderville, Curve Lake and Hiawatha First Nations, and urban/ rural Indigenous.
