Community Members!

Thank you for your interest in the LEKT Clinic project. We are working to build a new health and wellness clinic to provide accessible and inclusive health services to our community. Your feedback is crucial to help us understand what services and spaces would be most beneficial to you and your family.

This survey will take approximately 10 minutes to complete. Your participation is voluntary, and all responses will be shared anonymously. The survey includes 25 questions, some of which are optional.

Your contact information will be collected separately from your survey responses to ensure your feedback remains anonymous.
