As an organization that is run by and for patients, CAPA wants to hear from you! How well are we doing? What more can we do to meet your needs? You are welcome to complete this survey if you have an interest in our work. We want to hear from all of our supporters to help us figure out what to do next.

Please take this 10 minute survey and help us shape CAPA’s activities in 2025 and beyond. All of your responses are anonymous and will remain confidential.
