Survey Information for consent
This survey is going to ask for your knowledge, experience and attitudes regarding hormonal contraception. Please read the information below to decide if you would like to consent to participate.

To participate in the survey the following statements need to be true:
- You are over 16 years old
- You live in New Zealand

Purpose
We are curious about what people think about hormonal contraceptive methods and what experiences they have had using them. We want to hear your experiences using them, where you learn about them, and who influences your decision-making around whether to use them or not. The information gathered will be summarised in a report and shared with people who are interested in this topic. The content will also likely be included in other communications materials and social media posts.

Procedures

This online survey will pose multiple choice and open-ended questions. It is expected that the survey will take around 10 minutes.

Risks/discomforts
The risks in engaging in this survey are minimal. The survey includes questions about whether you know about hormonal contraceptive methods, your experiences using them, who influences your decision-making around contraception, and where you get information about such methods.

Benefits
There are no direct benefits to you, but your answers will help us understand more about what people know, experience, and feel about hormonal contraceptive methods.

Extent of Anonymity and Confidentiality
All your responses will be kept confidential. Your name will not appear anywhere in our research. No one will be able to trace your answers back to you. Each person who takes the survey will be assigned a number (e.g. 0210) which will be used to organise the data for the analysis. Select staff members from Sexual Wellbeing Aotearoa will have access to the data which will be used for a report summarising the findings and other possible academic publications. The data will be securely stored for a period of five years and then destroyed.

Voluntary Participation
You do not have to agree to take part in this survey. You may stop participating at any point during the survey or after the survey is complete, you may state that you do not want your responses to be included in the data. You also do not have to answer any questions in the survey for which you do not want to provide a response. You may contact Sexual Wellbeing Aotearoa at policy@sexualwellbeing.org.nz if you have any questions or complaints about being in this study or wish to have your responses removed from the data.
