WELCOME TO THE SURVEYThis survey was created to help our community better understand our residents' needs and the services required to improve and enhance community well-being.
Your answers will help us identify the types of services currently being accessed, which ones are currently seeing the most use, and what needs in the community aren’t being met. We will use the information gathered from this survey to take steps to increase and/or improve the services offered within our community.

- The survey is open from October 1 - October 31, 2025.
- If you have any questions about the survey, please reach out to staff at a participating service provider location for support.
- Time required: 15 minutes.
- This survey asks questions about your current and past living situations, employment, service use, and more.
- We do not ask for your name or contact information during the survey - our goal is to protect your privacy at all times. Your answers will be grouped with other survey respondents and reported in larger numbers. This means none of your answers will be linked back to you at any time.
- Your participation in this survey is completely voluntary - you are not required to answer any of the questions in the survey, and you are allowed to stop the survey at any time.
