Thank you for taking the City of Madison’s Impaired Driving Survey! This survey is for adults 18+, and will take about 10 minutes to complete. You have the option to share an email address or phone number to enter into a $25 gift card raffle.
Background and Purpose:
Impaired Driving is when someone operates a motor vehicle while affected by substances. The purpose of this survey is to help the City of Madison plan safety messages and ride share opportunities to reduce impaired driving locally and improve safe streets for all road users in Madison. We’re interested in your opinion on road safety risks from using various substances, what makes it hard to use a ride share or public transit, your ideas for a ride share program, and campaign strategies that connect with you.
Privacy:
This survey is anonymous. We do not collect IP addresses or names. We will not ask about your own use with substances. We will offer the option for you to share your email address or phone number only if you would like to enter into a $25 raffle. Your individual answers will be combined with hundreds of others to identify community trends, not individuals. To protect your privacy, the data we collect about areas in Madison where people live most of the time will only be reported if at least 10 people from that area of Madison take this survey. We will never report data for a single household or street.
