As we begin a strategic planning process to guide our next few years, we want to hear from our communities and learn more about your needs and opinions.

To help us do that, please complete this survey no later than 5PM on Friday, July 10th.

Responses are completely anonymous and go directly to our third-party facilitator, who will share key results with us later this summer.

PLEASE NOTE, though: In order to collect the most useful data possible, we need everyone responding to this survey to be in one of two groups:

1. People who live in one of the communities we serve: Chino Hills, Claremont, Covina, Diamond Bar, Glendora, La Habra, La Verne, Pomona, San Dimas, San Gabriel or Temple City;

OR

2. People who live outside those communities but have interacted with IVHS and SGVHS, perhaps through adopting a pet from us or using our low-cost spay/neuter or veterinary services.
