Phoenix Health & Safety and Health & Safety Matters are conducting an annual study into how organisations manage health and safety capability, competence and emerging risk.

We want to understand what is working, where organisations face pressure and how confidently current arrangements could withstand scrutiny.

Please answer with reference to the main organisation in which you work or provide health and safety support.

If you support several organisations, please select one organisation you know well and answer consistently with reference to that organisation.

The survey takes approximately eight minutes to complete. Responses will be analysed and reported in aggregate.

Phoenix Health and Safety is responsible for the information collected through this survey.

Results will be reported in aggregate, and written responses may be quoted anonymously. Health and Safety Matters will not receive any raw survey responses.

Participation is voluntary. Phoenix will not publish any personal or organisational identifying information within the survey results.

A member of the Phoenix Health and Safety team may contact you to inform you of services that may be of interest depending on your survey results.
