Once a year HPCN will ask parents and carer’s, to complete a single detailed survey. This completely anonymized data is then used to give a true picture to both Hampshire County Council (HCC) and the local NHS : Hampshire ICB, at what things really look like.
The answers will shape HPCN’s focus for the next year. Some of the anonymous data will go directly into the shared self evaluation form, for Hampshire County Council, and Hampshire Isle Of Wight Integrated Care Board, which OFSTED and the CQC judge them on.
It will also drive the Special Educational Needs and Disabilities (SEND) and Alternative Provision (AP) Local Area Inclusion Plan, and the combined HCC & ICB Self Evaluation Form. Key items in the next combined OFSTED and CQC inspection.
While Hampshire County Council and other partner agencies may promote this survey and ask you to complete this, HPCN will only share with them the anonymized processed data where relevant. HPCN will be the sole data controller and processor.
If you have more than one child, please feel free to complete this for each child if you would like. If you are completing on your own behalf, where questions say your young person, please fill out for yourself.
