This short survey will ask for more information about device access for your student.
You'll decide whether you'd like your student to use a school device at home. Depending on the grade level, the frequency might vary. Intermediate students might do so from time to time.

If your student is in grades 10-12, your student may already have a device and you'll be agreeing to continue.
You'll complete the form for each student. At the end, you'll be directed back to complete it for another student.
