Introduction

 

Aloha Coffee Farmers and Processors,

The Hawaii Department of Agriculture (HDOA) Coffee Berry Borer (CBB) Pesticide Subsidy Program was established during the 2014 Legislative Session by Act 105 (HB1514) to assist Hawaii coffee farmers with the cost of pesticides containing Beauveria bassiana (Botanigard® and Mycotrol®) for CBB control. The program is funded through 6/30/2019 in cooperation with the County of Hawai`i.

Applications are currently accepted for purchases made between July 1, 2015 and June 30, 2016. If you are applying for the subsidy, please complete Sections I and II. Section I is Farm Owner/Manager Information and Section II is Program Impact. If you are not applying for the subsidy, you have the option to skip to Section II to answer questions about CBB.

When you start the application you cannot save your responses and return at a later time. If you would like to preview the questions, email hdoa.cbb@hawaii.gov and request a copy of the application questions. It will take about 25 minutes to complete. You will need your contact information, tax map key (TMK) number, CBB damage, your Marketable Green Bean Recovery Ratio (MGBRR)*, CBB management strategies and other information about your farm. If you own or manage more than one farm, one application is required for each tax entity. Once you have answered all questions click the "DONE" button to record your responses. After completing the online application, you will be contacted by the CBB Pesticide Subsidy Program office to schedule an appointment. Bring these required documents to the appointment: original receipts, an invoice from your farm, and a Driver's License or State issued ID. At the appointment, a County of Hawaii Form V-8 will be provided for you to complete with your State of Hawaii General Excise Tax ID number and your Federal Tax Identification Number or Social Security Number. If you have questions contact the CBB Program office at 808-323-7578 or email at hdoa.cbb@hawaii.gov.

*MGBRR is the weight of cherry harvested during the season divided by the weight of marketable green bean from that season. If you sell cherry you may not have this number.

I understand HDOA requires original documentation of costs for purchasing pesticides that contain Beauveria bassiana, a fungus known to control the coffee berry borer, and that HDOA may contact me with important information about CBB. Information gathered in Section II for Program Impact will not be shared on an individual level; only summarized results will be shared with the State Legislature, CBB Task Force, UH-CTAHR, and coffee organizations.

Further, I shall indemnify and hold harmless the State and the County of Hawaii, and its officers, agents, and employees from all claims arising out of or resulting from the pesticide purchased.
