About the Survey
Residents aged 16 and older are invited to complete the Community Social Services and Supports Survey from September 14 to October 14, 2026. The confidential survey explores social issues, programs, and services that support individual, family, and community well-being.
Feedback will help the County and Family and Community Support Services shape future programs and supports. The survey takes about 20 minutes to complete, and participants can enter a draw for a $300 grocery gift card.

The information collected as a part of this survey is for the purposes of informing local community programs and social policy. Written statements may be included in final report provided they do not include information that may identify individuals. All personal information collected is for the purpose of this study and is collected under the provisions. Personal information collected on this form will be used for the purpose of County of Grande Prairie No.1 FCSS Community Needs Assessment Survey. This collection is authorized by Section 4(c) of the Protection of Privacy Act and the information collected will also be subject to the Access to Information Act.
