2026 Licking County Provider Nominations

The 2026 award categories have changed. There will be 5 awards, one award will be given for Supervisor of the Year, Independent Provider of the Year, Employment Specialist of the Year, HPC-DSP of the Year, and ADS-DSP of the Year. Please read all award descriptions below and choose the best ONE that fits for your nomination. We encourage you to give as much detail as you can, so the voting committee can get to know the candidate as much as possible. Please only include one person for each nomination, group nominations will not be considered for an award!
SUPERVISOR OF THE YEAR AWARD: This supervisor/manager/leader helps others excel and provides opportunities for growth/advancement of staff and demonstrates leadership that fosters collaboration.

INDEPENDENT PROVIDER OF THE YEAR AWARD: This independent provider connects the individual(s) they support with their interests in the community, building new relationships and gaining independence.

EMPLOYMENT SPECIALIST OF THE YEAR AWARD: This employment specialist helps develop employment skills and match employers to an individual's interests. This ultimately allows people to earn an income doing something they love.

HPC-DSP OF THE YEAR AWARD: This HPC-DSP provides one-on-one support in an individual’s home, assisting with daily tasks, building routines, and enhancing their comfort, dignity, and quality of life.

ADS-DSP OF THE YEAR AWARD: This ADS-DSP helps individuals build skills, engage in activities, and live more independent, fulfilling lives while providing compassionate, person-centered support throughout each day.
