Golden Heart Award Nominations Survey

This prestigious award is for any individual who has "opened their heart" to touch the life/lives of a Cherry Creek School District student(s) with special needs. They redefine what it means to serve students with special needs in Cherry Creek School District by seeing a need and meeting it with heart, creativity, and unwavering dedication.

IMPORTANT: Nominations submitted need to be specific and detailed AS TO HOW THE NOMINEE GOES ABOVE AND BEYOND for students with special needs in Cherry Creek Schools.

Some examples of nominations are:

~ Bus driver who builds trust... tell how;

~ Teacher or paraeducator who became a lifeline... tell the specific story;

~ School staff member who creates a safe and inclusive space by...;

~ Specials teacher, who modifies curriculum and creates opportunities for inclusive participation (media, art, P.E., music) by...;

~ Student who is a leader in inclusion at the school... tell how;

~ Community member or business who provides opportunity for CCSD students... explain what they do...;

IMPORTANT TO KNOW AND CONSIDER:
~ ALL NOMINATIONS MUST BE RECEIVED BY 4PM on December 12th
~ Each person (or family) may nominate one individual for the award. NOTE: You may not nominate a member of your own family.
~ Nominations for an entire team cannot be accepted. The number of nominations submitted for each person does not increase a nominee's chance of being selected.
~ CCSD honorees must be in good standing with the district.
~ Honorees are chosen by an independent reading committee.

This electronic form should take about 15-30 minutes to complete and MUST BE COMPLETED IN ONE SITTING. Incomplete nominations will not be accepted.

Golden Heart Award recipients will be notified in February if they have been chosen to receive the award.
