Primary Care Engagement Strategy Health Professional Survey

Thank you for participating in this survey. The survey is for GPs, nurses and nurse practitioners working in primary care.

Dementia Australia is developing an organisation wide Primary Care Strategy aiming to strengthen Dementia Australia’s impact within the primary care system, including with GPs and practice nurses.

Dementia Australia will use the information you provide in your survey response to inform our strategy and priority actions for working with primary care. We will also use de-identified responses in our advocacy to support policy changes that improve people living with dementia's experience of primary care.
