Welcome to this survey on Sustainable Island Futures on Prince Edward Island

Dear Sir or Madam,

My name is Dr. Jim Randall. I am the Chair of the Institute of Island Studies (IIS) at the University of Prince Edward Island (UPEI) in Canada and the lead researcher for this project titled “Comparing small island states and subnational island jurisdictions: Towards sustainable island futures”.

I am partnering with my colleague Dr. Romina King in the Geography Department at the University of Guam to better understand the role that governance plays in achieving a more sustainable future on your island. This research is funded by the Canadian Social Sciences and Humanities Research Council and is being carried out by approximately 600 people on 12 islands around the world. This Consent Form describes the purpose of the survey, your rights and our responsibilities. You have been selected because you fall into one of the following groups: a member of a non-governmental organization, government, youth, business, worker/trade union, or an academic.

This survey consists of 19 closed-ended questions. It should take you no more than 30 minutes to complete and carries with it minimal risk to you. Your responses will be returned only to me and to Dr. King. They will not be shared with anyone else in their raw form. Your responses will be kept confidential to us and to the limits of the law and will be stored on my desktop computer’s password-protected hard drive in my locked office. After five years the data will be erased. The online survey is administered by SurveyMonkey, an American software company. As such, your responses are subject to US laws, including the Patriot Act. Although we cannot guarantee the confidentiality of your responses, the risks associated with participation are minimal and similar to those associated with email programs such as gmail and social media platforms such as Facebook or MySpace.

The questions in this survey carry no more than minimal risk to you. As a volunteer, you can skip any questions and can stop at any time without repercussions. Even after you have completed the survey, if you want your answers excluded from the results all you need to do is contact me at jarandall@upei.ca within one month of completing the survey. None of your individual answers will be shared with anyone. Any final reports, research articles or conference presentations will only use grouped responses. Your continued confidentiality will be protected for all of these uses.
