2025 Fall/Winter Parent Survey

This survey is completely voluntary and anonymous. All information will help inform the work of Building a Positive Community, the Deerfield Valley Community Partnership and the West River Valley Thrives (Thrives).
This survey should only take a few minutes to complete.
1.For the purpose of this survey, please choose one child in grade 6th-12th to reflect on. For the current school year, what grade is this child attending?(Required.)
ALCOHOL -  When questions refer to drinking alcohol, this includes drinking beer, wine, wine coolers, rum, gin, etc. For these questions, drinking alcohol does NOT include drinking a few sips of wine for religious purposes.
