INTRODUCTION


You are invited to take part in this pilot survey about your experiences in relation to functional neurological disorder [FND]. 

[NOTE: This condition has historically been known by a number of other terms including Conversion Disorder. This survey also includes those who are diagnosed with Functional Seizures, also referred to as psychogenic non-epileptic seizures or Non Epileptic Attack Disorder. For the purpose of this survey we are including consideration of co-existing symptoms such as chronic pain & fatigue.]

PURPOSE

A collaboration of four FND Charities: FND Action, FND Dimensions, FND Friends and FND Australia Support Services has been formed.  We are  working collaboratively to advance FND in Australia and the UK.

The survey is a brief pilot, with a plan that the outcomes will feed into a larger evidence based study with our University partner.  The brief survey will capture the experiences of individuals in Australia and the UK  who live with FND.

All information collected will be aggregated.  All responses are anonymous and no identifying information will be collected as part of the survey.

THIS SURVEY SHOULD TAKE NO MORE THAN 5-10 MIN TO COMPLETE.  You agree to participate in the survey voluntary.  If you decide not to participate, please close your window prior to submitting your survey responses.

LOOK AFTER YOUR SELF

FND/CD can be a debilitating and distressing illness. It is possible that thinking about your experiences may bring up some emotions or difficult feelings. Please take your time in completing the survey, self-care and reach out for support as needed.
